The Social Worker provides social work services for patients and their families.


 All duties will be performed per scope of practice and clinic policy, in accordance
with regulatory agencies and organizational/departmental guidelines.
 Assesses situations, plans and coordinates for patient needs, initiates a
treatment plan individualized to needs of each client, implements plan
considering the least restrictive environment and reasonableness of effort,
carries out the plan and evaluates its effectiveness through the course of
treatment up to and including discharge, as may be consistent with his/her level
of professional training, experience, and skills.
 Documents all treatment services provided to patients in the patient’s service
 Provides emergency/crisis services to patients as assigned or as necessary and
documents emergency/crisis services provided.
 Makes an initial visit with patient and family and subsequent follow-up visits
according to established company policy and procedure.
 Address individual patient goals as well as concerns outlined on the care plan,
making note of problems resolved or new problems added.
 Assist patient and family with living arrangements as well as nursing home
placement as appropriate.
 Must be able to perform the physical demands of the job.
 Maintains regular and reliable attendance.


 Holds at least a Bachelor’s Degree in Social Work from an accredited university
or school of social work.
 Holds a current Iowa license as a Licensed Independent Social Worker, Licensed
Social Worker, or Licensed Mental Health Counselor or will be eligible to obtain
licensure within agreed time period.
 Required to be current in BLS (Basic Life Support) by the standards of the
American Heart Association (*not applicable to Hospice).
 Must adhere to all training requirements as mandated by Federal, State, and
Professional licensure and regulatory standards.
 Required to show proof of having completed Mandatory Reporter Course at time
of hire or attend within 6 months of start date.

 A belief in the hospice philosophy.
 Prior hospice training preferred.
